Inspection Application Forms

The applications below are to be used when applying for an inspection. You may also find the Fee Estimator and accompanying application form attached to it helpful when applying for inspection. Please complete the form and submit it to the Electrical Safety Authority.

Please note that application forms cannot be submitted directly through the website to our customer service centre. You can fax your application to 1-800-667-4278 or you can email your application to esa.cambridge@electricalsafety.on.ca. For inquiries please call 1-877-ESA-SAFE.

Note that there are 3 different Residential Application Forms below. Please ensure you choose the one that best suits the type of electrical inspection you will require: 1) New Residential, 2) General Inspection, or 3) Residential Renovation.

Emailing ESA Inspection Correspondence
If you are an account customer, you can call us to go paperless and receive Certificates of Inspection, Outcome Summaries, and Defect Notices by email. Please contact the Customer Service Centre at 1-877-ESA-SAFE (1-877-372-7233) and select option #4 to make arrangements to have ESA email your correspondence.

Please Note: If you are an Authorized Contractor Program participant, please continue to use the application form under the Authorized Contractor Program section of this website.
